To open a DDF cheque account download the appropriate forms as detailed below. Complete and return the original forms to the DDF for processing.
1. Cheque Account Application
The cheque account application must be authorized by a parish priest, school principal, superior, etc.

2. Group B Signatory Authority
Please have all persons to be authorised signatories complete the sections of the Group B form as detailed below:
Date (write the effective date) top right hand corner of the form
Authorised Signatories Names in Full (print each signatory’s name in full) bottom of page – middle column
Specimen Signature (each signatory to sign) bottom of page – right hand side

3. Identification Reference Form
Each new signatory to a DDF account needs to provide 100 points of identification. Usually a Passport or Birth Certificate, plus a Drivers Licence are the easiest forms of identification. New signatories can come into the DDF office with their identification or an Acceptable Referee can identify the signatory.
The Acceptable Referee must sight identifying documents and sign the form. They must have known the person being identified for at least 12 months. Please return the completed forms (original forms only) to the DDF for processing. PO Box 2605, North Parramatta NSW 1750 When we’ve opened your account we’ll send you a cheque and deposit book. Further books can be obtained by contacting the DDF.
